Are you an Alcoholic?

Feel free to print these questions, for yourself, your spouse, or for someone you know. 

This test can apply to all forms of substance abuse.

To answer this, ask yourself the following questions and answer them as honest to yourself as you can.


(The test questions below are the same that are used by John Hopkins University Hospital)

1.  Do you lose time from work due to substance abuse?   Yes  __   No __

2.  Is substance abuse making your home life unhappy?   Yes  __   No __

3.  Do you abuse substances because you are shy with other people?   Yes  __   No __

4.  Is substance abuse affecting your reputation?   Yes  __   No __

5.  Have you ever felt remorse after abusing?   Yes  __   No __

6.  Have you gotten into financial difficulties due to substance abuse?   Yes  __   No __

7.  Do you turn to lower companions and an inferior environment when abusing?   Yes  __   No __

8.  Does your substance abuse make you careless of your family's welfare?   Yes  __   No __

9.  Has your ambition decreased since you started abusing substances?   Yes  __   No __

10.  Do you crave substances at a definite time daily?   Yes  __   No __

11.  Do you want to abuse the next morning?   Yes  __   No __

12.  Does substance abuse cause you to have difficulty sleeping?   Yes  __   No __

13.  Has your efficientcy decreased since you started abusing?   Yes  __   No __

***Apply your honest results to the information below:

If you have answered 'Yes' to any one of the questions, there is a definite warning that you may be an alcoholic.

If you have answered 'Yes' to any two questions, the chances are that you are an alcoholic.

If you have answered 'Yes' to three or more questions, you are definitely an alcoholic.
